Does anyone recognize this with Queen Elizabeth II on it

I think this is likely a token or a medal it has most definitly be made into a pendantat some time.
It was very crudely plated in what I assume is silver.
I do not even recognize the portrait of the Queen or the coat of arms on the other side
Hopefully someone else will no what this is

It is non magnetic and 15.5 mm in diameter
