I think it is because the machine that punches out the holes was not set up correctly.
Though I would have thought it would have been easier to stamp them (coin AND hole) out all in one go;
I say that as I assume it is a two-part process. The whole coin is made, then the centre hole is made separately. I have in the past seen pictures of holes that are wildly off-centre.
I also have an off-centre coin. Mine is from Denmark (first picture). Then two other images I found, especially the last one! ...
