Does anyone know what this is? I've been trying to identify it but unable to find the exact same coin.

diameter: 27mm
thickness: 1.5mm
weight: 9.5g

It's a follis (big M) minted in the third officina (gamma under the M) of Constantinopolis (CON in exergue) during the 4th year (ANNO II - II in the fields) of the reign of Heraclius and Heraclius Constantine (his son, as co-emperor).
